First, we want to get to know you, so. . .
We ask all new patients to complete the patient registration forms so we can learn more about your goals for treatment and share information about our office policies and procedures.
If you prefer to type your responses, please complete the Fillable forms and email to: Wellness@OptimaLivingTherapy.com. If you are not able to complete the form electronically or prefer to hand-write, download the Print/Write forms to complete by hand and bring them with you to your first appointment.
If we are seeing your for a pelvic floor concern, also complete the appropriate Women's or Men's Pelvic Health Questionnaire.

We want you to be comfortable, so. . .
Wear or bring loosely fitting clothing for the examination - such as shorts and a tank-top or t-shirt. This allows us to easily see and assess the affected body part. You can bring clothes with you to change before your appointment. We have gowns available, but most people find it more comfortable to be in their own clothes.
Bring cash, credit card or check with you. . .
Even if you have insurance, most plans stipulate a co-pay of some kind. Payment of co-insurance is due at the time of service. By law, we are unable to waive co-pays or deductibles.
NOTE about Insurance: As a courtesy, we will attempt to verify your deductible and other benefit information. However, we are not able to guarantee the insurance company will provide accurate or up-to-date information. It's our experience that insurance companies provide more reliable information to their members than to providers.

IMPORTANT: One word of warning - if your insurance company tries to discourage you from coming to our clinic because we "don't participate" with your particular plan, don't worry. They're just doing their job. They save money if you use an in-network provider. But in most cases, the cost is no greater for you to come to us for top quality care. In addition, because we are the only pelvic health specialists in the West Valley, you may be able to request our services be considered "In-Network". Our Clinical Director can help you with this process following your evaluation.
Lastly. . .
- We kindly ask new patients to arrive 15 minutes before your evaluation appointment so we can complete your registration process.
- Also, bring any x-rays, MRIs, or diagnostic reports that relate to your condition so we can use this information to determine the best plan of care.
